A 38-year-old man with skin rash, shortness of breath, cough, and fever.
Imaging Findings
(A, B) Computed tomography of the chest without contrast reveals ill-defined ground-glass nodules scattered throughout both lungs (arrows). No lobar consolidation, no pleural effusion, and no enlarged lymph nodes are noted.
Differential Diagnosis
• Varicella pneumonia: The association of the typical skin rash with multiple pulmonary nodules scattered throughout the lung parenchyma should raise the question of varicella-zoster virus pneumonia.
• Other viral pneumonias (cytomegalovirus [CMV] pneumonia, influenza virus pneumonia, measles):
