Clinical Presentation
A 66-year-old man who has received a right-sided lung transplant for pulmonary fibrosis presents with right-sided chest pain, fever, and cough.
Imaging Findings
(A, B) Contrast-enhanced computed tomography (CT): Axial (A) and coronal (B) images demonstrate a focal area of air-space nodular consolidation in the right lower lobe, surrounded by a ground-glass halo of lesser density. An irregular wedge-shaped opacity in the periphery of the middle lobe is also seen (arrows). There is end-stage fibrosis of the left lung.
Differential Diagnosis
• Angioinvasive aspergillosis with the halo sign: In an immunosuppressed patient, a dense nodule surrounded by a ground-glass halo is highly suggestive of angioinvasive aspergillosis.
