Case History
A 54-year-old woman presents with new right breast lump. Twelve years ago, she had a right modified radical mastectomy for extensive ductal carcinoma in situ. She subsequently had breast reconstruction with placement of a permanent implant.
Physical Examination
• right breast: a hard mass at the 5:00 position; 3 cm palpable node in the right axilla
• left breast: normal exam

• margin: circumscribed
• shape: lobular
• density: high density
