FINDINGS Figure 219-1. Sagittal post-contrast T1WI shows a peripherally enhancing lesion in the pituitary gland (vertical arrow) with thickened and enhancing stalk (transverse arrow). Figure 219-2. Coronal post-contrast T1WI through the sella turcica shows the lesion extending into the suprasellar cistern and touching the chiasm (arrow). Figure 219-3. Corresponding coronal T2WI. There is hypointensity of the periphery of the lesion (arrows) enclosing a hyperintense core. Lack of cavernous sinus involvement and mass effect on the optic chiasm are better seen on this sequence.


DIFFERENTIAL DIAGNOSIS Pituitary hyperplasia, pituitary adenomas, suprasellar tumors (germinoma or astrocytoma), inflammatory pseudotumor, sarcoidosis, and histiocytosis.


DIAGNOSIS Lymphocytic hypophysitis (LYH).
