There is an enlarged, elongated T2 hyperintense structure (isointense to endolymph of adjacent inner ear structures) along the posterior margin of the right temporal bone consistent with an enlarged endolymphatic sac (long white arrow). The diameter of the right endolymphatic duct/sac is larger than the adjacent semicircular canal (short white arrow). The normal left endolymphatic duct/sac is not visible on MRI, an expected finding.
DIFFERENTIAL DIAGNOSIS Pendred syndrome, endolymphatic sac tumor, enlarged endolymphatic sac.
DIAGNOSIS Enlarged endolymphatic sac syndrome.
