FINDINGS Figure 289-1. Axial post-contrast T1WI through the posterior fossa. There is a large mass arising from the left cerebellum, compressing the brainstem and effacing the fourth ventricle. The dominant features of the mass include a large cyst (black arrow) and a mural nodule (white arrow), which are classic imaging features of pilocytic astrocytoma (PA). Smaller cysts and septations with mural enhancement are also seen in the posterior aspect of the mass.


DIFFERENTIAL DIAGNOSIS Ependymoma, medulloblastoma, choroid plexus papilloma, hemangioblastoma, PA cystic metastasis, and infection such as toxoplasmosis.


DIAGNOSIS Pilocytic astrocytoma (PA).
