A patient scheduled for a Tc99m-MDP bone scan was dosed with 100 mCi of radiotracer instead of the prescribed 20 mCi. The patient’s estimated whole-body dose is 1.3 rem, and the highest organ dose is 5 rem to the bone. Is this event reportable?
What should be done now?
• This is a “recordable” event: Correct, because the administered dose differed from the prescribed dose by > 20%, but the total-body dose to the patient was < 5 rem (50 mSv) and the highest organ dose was < 50 rem (500 mSv). This event must be recorded locally, and documentation must be prepared that determines how this occurred and what steps have been taken to prevent it in the future. It does not have to be reported to the Nuclear Regulatory Commission (NRC), however.
