A 77-year-old woman presents with intermittent right lower quadrant pain.

Contrast-enhanced computed tomography (CT) shows contrast within the terminal ileum abutting a fat-density mass (arrow) that projects into the lumen of the cecum.

Differential Diagnosis

• Ileocecal valve lipoma: This is the top diagnosis, indicated by a focal, fat-density mass that is not continuous with mesenteric fat.

• Lipomatous infiltration: This may have a similar appearance of fat density within the ileocecal valve.
