Clinical Presentation

A 35-year-old man presents with fever, chronic shortness of breath, and occasional hemoptysis.

(A) Thoracic angiogram shows a small vessel (arrow) off the aorta, just above the left hemidiaphragm. (B) Selected angiogram of this vessel shows branches (arrow) supplying a region of the left lower lobe. (C) Venous phase shows pulmonary venous (arrow) drainage.

Differential Diagnosis

• Intralobar pulmonary sequestration: Indicated by a left lower lobe structure with systemic artery supply and pulmonary venous drainage.
