Imaging
CT or MR evidence of pancreatic tissue and annular duct encircling descending duodenum
Large pancreatic head encircling 2nd duodenum (marked by gas, enhanced mucosa, or oral contrast)
Fluoroscopic barium study (UGI)
Concentric narrowing of 2nd part of duodenum ± dilated proximal duodenum
ERCP or MRCP
Duct draining pancreatic head encircles duodenum
Top Differential Diagnoses
Postbulbar peptic ulcer
Similar findings on UGI but not on CT or MR
Pancreatic ductal carcinoma
Can mimic annular pancreas on UGI or CT
Duodenal carcinoma
Attenuation of duodenal carcinoma is usually different than that of pancreas
Pathology
Associated congenital anomalies in up to 75% of pediatric cases
e.g., atresia of esophagus, duodenum, or anus
Clinical Issues
50% of cases present in infancy
