29 Gastric outlet obstruction Fig. 29.1 This radiograph demonstrates a distended stomach and a paucity of bowel gas elsewhere. This patient has gastric outlet obstruction (strictly speaking in this case the obstruction is probably just distal to the stomach as gas is seen in the duodenal cap). Note the jejunostomy and right total hip replacement. Gastric outlet obstruction is defined as any process that impedes emptying of the stomach. Its causes are similar to those of intestinal obstruction – functional and mechanical. Diabetic gastroparesis and ileus/pseudo-obstruction are functional causes of obstruction. Mechanical causes can be divided into benign and malignant. The commonest benign cause is peptic ulcer disease leading to scarring and pyloric stenosis. Pancreatic pseudocysts can also cause the same appearances. Malignant causes include pancreatic and gastric tumours as well as metastases. Clinical features Symptoms Patients present with non-bilious vomiting, often shortly after ingestion of a meal, with relatively undigested food in the vomitus. Weight loss may be a feature.
