Points to consider
Technique
PA – radiograph must be taken on full inspiration
PA – possible pneumothorax – radiograph taken on full expiration
Make sure the scapulae are clear of the lung fields
If the patient cannot place their hands on hips – turn the hands internally and place the arms around the cassette
Radiological assessment
Poor inspiration may mimic cardiac enlargement
Small pneumothorax best seen on an expiration radiograph
Full inspiration – right 8–9 posterior ribs are just clear of the diaphragm
Haemoptysis – look for evidence of lung cancer or tuberculosis
Breathlessness – look for heart failure or collapse of lobes
PA
• Patient facing the cassette with chin extended
• Trunk is adjusted so that the median sagittal plane is perpendicular to the cassette
• Feet are parted to maintain stability
• Dorsal aspect of the hands are placed behind and below the hips and the elbows are brought forwards
• Shoulders are relaxed and rotated forwards until they are in contact with the cassette
• Instruct patient not to raise their shoulders as they breathe in
