Abstract
Waldenstrom macroglobulinemia is a low-grade lymphoid malignancy composed of mature plasmacytoid lymphocytes with production of abnormal monoclonal IgM protein. A macroglobulin proteinaceous hyaline material fills lacteals in lamina propria of the small bowel with lymphatic distension and edema.

Waldenstrom macroglobulinemia is a low-grade lymphoid malignancy composed of mature plasmacytoid lymphocytes with production of abnormal monoclonal IgM protein. A macroglobulin proteinaceous hyaline material fills lacteals in lamina propria of the small bowel with lymphatic distension and edema.
Mean age at diagnosis is 60 years old; symptoms are fatigue, weight loss, diarrhea, steatorrhea, malabsorption, and anemia. Most important for diagnosis is the IgM elevation with hyperviscosity syndrome (bleeding, visual changes, neurologic abnormalities).
The small bowel is rarely involved with bowel dilatation and uniform diffuse thickening of valvulae conniventes.
