Clinical Presentation
A young child presents with hypothyroidism.
Anterior and lateral views from an iodine 123 scan demonstrate a focus of activity in the region of the tongue base (circle) and no activity in the thyroid bed. Markers have been placed on the chin (CHIN) and suprasternal notch (SSN).
Differential Diagnosis
• Lingual thyroid: A focus at the tongue base with lack of uptake in the expected thyroid bed is diagnostic.
• Hyperfunctioning thyroid adenoma: This can also appear as a solitary focus with nonvisualization of the remainder of the (suppressed) gland, but it is located in the thyroid bed and typically in clinically euthyroid or hyperthyroid adults (see Case 23).
