Clinical Presentation
A 72-year-old man with chest pain undergoes rest–stress myocardial perfusion scintigraphy.
(A) Anterior projection of the raw data from a resting myocardial perfusion scan with thallium 201 demonstrates normal uniform uptake throughout the liver and left ventricular myocardium (arrow). However, an abnormal focus of activity is seen more superiorly in the left paramediastinal location (circle). (B) Anterior projection of the raw data from the stress myocardial perfusion portion with Tc99m-TETRO demonstrates normal uniform uptake throughout the liver and left ventricular myocardium. Also note the normal biliary/gallbladder (GB) excretion (arrow) that is seen with TETRO and MIBI, but not with thallium. An abnormal focus of activity is again seen more superiorly in the left paramediastinal location (circle).
