Enchondroma versus Chondrosarcoma


Enchondromas and low-grade chondrosarcomas may be difficult to differentiate by imaging (Table 121.1). The presence of pain related to the lesion is helpful as almost all chondrosarcoma patients present with pain. However, enchondromas can be seen as an incidental finding on imaging in patients with musculoskeletal pain from other etiologies. Location is very helpful as chondroid lesions in the axial skeleton are highly likely to be malignant and lesions in the hands and feet are unlikely to be malignant. Usually, lesions in the long tubular bones are difficult to differentiate.1
