Clinical Presentation


A 3-year-old child is admitted to the hospital with rectal bleeding.

Technique


• Cimetidine (20 mg/kg) is administered orally for 24 to 48 hours before the study.


• The patient is fasting for 4 hours before the radiopharmaceutical is injected.


• The patient should void before the study. Catheterization may be required.


99mTc-pertechnetate (0.1 mCi/kg) is administered intravenously.


• Sequential 5-minute images are acquired for 1 hour.


• Upright and oblique static images can be obtained as necessary to confirm localization or differentiate gastrointestinal activity from urinary tract activity.


Image Interpretation


Serial 5-minute anterior images over the abdomen (Fig. 146.1A,B) show a progressive focal increase in tracer accumulation in the right lower quadrant, although it appears decreased and blurred in the last few frames. A single anterior 10-minute image (Fig. 146.1C
