96 Almost all fat-containing renal masses are angiomyolipomas. Fat density has been reported in renal cell carcinomas, but this is extremely rare. However, a mass that contains both fat and calcification is suspicious for renal cell carcinoma as angiomyolipomas rarely contain calcification.1 Fat density without calcification has been reported in papillary renal cell carcinoma, secondary to adipose tissue1 and cholesterol necrosis.2 Papillary renal cell carcinomas tend to be more homogeneous and enhance less than clear cell carcinomas.3 Clear cell renal carcinomas often have intracytoplasmic fat, which can be detected by chemical shift magnetic resonance imaging (MRI),4 but do not have macroscopic fat detectable by computed tomography (CT).
