Clinical Presentation
A 62-year-old woman who underwent computed tomography for nonspecific abdominal pain.
Imaging Findings

Contrast-enhanced computed tomography (CT) of the abdomen at the level of the adrenal glands shows a large mass splaying the limbs of the left adrenal gland (arrows). Soft tissue (asterisk) and fat-containing areas (arrowhead) are seen within the mass.
Differential Diagnosis
• Adrenal myelolipoma: A mass splaying the limbs of an adrenal gland suggests that the mass is arising from the adrenal gland. Macroscopic fat in a mass arising from the adrenal gland is pathognomonic for myelolipoma. The soft-tissue, non–fat-containing elements vary in amount and represent bone marrow.
• Angiomyolipoma from the upper pole of a kidney:
