Abstract

In its upper part, the mesovarium and the mesosalpinx are very thin, composed almost exclusively of peritoneum.

31.1 Embryology, Anatomy, and Histology

31.1.1 Embryology [1]

See Chap. 1.

31.1.2 Anatomy [2, 3]

See Chap. 2.

31.1.3 Histology

1.

In its upper part, the mesovarium and the mesosalpinx are very thin, composed almost exclusively of peritoneum.

2.

In the mesometrium, beneath the peritoneum, dense conjunctive fascicles are associated with muscular tissue and adipose tissue.

31.2 WHO Classification of Tumors of the Broad Ligament and Other Uterine Ligaments [4]

There is a broad variety of tumor and tumor-like lesions of the broad ligament (Table 31.1). Simple cysts (often called para-ovarian cysts) are very common and most often correspond to tumor like-lesions (cysts of mullerian, mesonephric or mesothelial origin) rarely to true neoplasms. In the absence of papillary projections or solid components, differentiation of a tumor-like lesion from a true neoplasm can be difficult [4, 5]

Table 31.1
WHO classification of tumors of the broad ligament and other uterine ligaments

1. Epithelial tumors

Mullerian

Serous tumors (cystadenoma, borderline cystadenoma, serous carcinoma)

 Mucinous carcinomas

 Endometrioid tumors

 Clear cell carcinomas

 Brenner tumor

Wolffian

 Papillary cystadenoma

Ependymoma

2. Mesenchymal tumors

Benign   leiomyomas, lipomas

Malignant  sarcomas (leiomyosarcomas and other sarcomas)

3. Mixed epithelial-mesenchymal tumors

Adenomyomas

Adenosarcomas

4. Miscellaneous tumors

Germ cell tumors

Granulosa cell tumors, thecoma, fibroma, steroid cell tumors

Adenomatoid tumor

Pheochromocytoma

5. Secondary tumors

Carcinoma

Lymphoma and leukemia

6. Tumor like lesions

Cysts

 Mullerian

 Wolffian

 Mesothelial

31.3 Imaging Findings

31.3.1 Common Topographic Characters in Favor of the Location to the Broad Ligament

These findings are reported in Table 31.2.

1. Location in the mesovarium or the mesosalpinx

Cystic mass separated from the ovary

Round shape, different from a hydrosalpinx

2. Location in the mesometrium
